We understand this is a very difficult decision to surrender your dog to us, and we are thankful you've entrusted us to find them your forever home for you. We have a few requirements before we can accept your pet, so please fill out the form below to submit your surrender request.

Please fill out all of the form. Filling it out with information on bite/injury history, or medical history will NOT hinder our decision to accept your dog. We need as much info as possible to be able to make the best choices for the animal.
